Description	This file appears to be a set of the papers used by nurses for the 1999 data collection of the MRC National Survey of Health and Development (NSHD), when they visited study members to gather data from them. File contains the following documents: - a nurse appointment card; - a diet diary; - question codes on a card, presumably for use with the CAPI (Computer-Assisted Personal Interview); - a brochure for the National Centre for Social Research (formerly SCPR); - a ‘Study of genes in the National Survey of Health and Development’ booklet; - a briefing example of the address record form, and a blank version; - appointment sheets; - a blood pressure measurement sheet; - an ‘Information Sheet for the 1999 Medical Examination’, to give to study members; - a ‘Consent Booklet 1999’; - a ‘Cigarette Brands Coding List’ booklet.
