Title	The Family Planning Study: Questionnaire F1

Description	The questionnaire used in a study on family planning. This was a sub-study of the National Survey of Health and Development (NSHD), and gathered data by carrying out interviews with the mothers of NSHD study members in 1960. The study was also sometimes referred to as the ‘Fertility Survey’. File contains a blank copy of the questionnaire used in 1960, which was know as ‘F1’, and two drafts of the questionnaire.
