Title	Second Generation Study: Blank forms, leaflets, and template letters

Description	Various template letters and forms, mostly taken from a folder entitled ‘Originals of documents’. These forms were used in the Second Generation Study, a sub-study of the MRC National Survey of Health and Development (NSHD). This file contains: - introductory booklets to the NSHD and the Second Generation Study; - template letters to study members as part of the Second Generation Study; - a Second Generation Study distribution sheet; - confidentiality forms for interviewers; - template thankyou letters for participants; - template letters to directors of education authorities; - template letters to hospitals and medical records officers; - workflow diagrams; - template letters to headteachers; - blanks consent forms, for parents, for the NSHD to be able to contact schools with questionnaires.
