Title	Maternity Survey questionnaires

Description	Blank questionnaires used for the maternity survey in 1946, which later went on to become the National Survey of Health and Development (NSHD). Two questionnaires were used for the maternity survey, which would have been completed by a nurse while interviewing the mother. This was a study of all births in the same week in March 1946, and the interviews with the mothers took place over April-June.
