Title	'Second Follow-up Survey' questionnaire

Description	The blank questionnaire used for the 1950 follow-up survey to the 1946 maternity survey, which later went on to become the National Survey of Health and Development (NSHD). This questionnaire would have been completed by a nurse while interviewing the mother, and it asked about the baby's health and development. The interviews took place in June.
