DescriptionForms containing lists of mothers who had given birth in one week in March 1946, compiled for a nationwide survey of maternity. The mothers on these lists were then visited by a health worker for a questionnaire interview several weeks after giving birth. This study later went on to become the MRC National Survey of Health and Development (NSHD), collecting data at regular intervals from a sample of the babies born in this week in 1946.

These forms are arranged by Local Authority Number, which range from 1 to 460. Each form is a list of new mothers in that particular local authority, using the following columns:

- ‘Mother’s Name’;
- ‘Mother’s Address’;
- ‘Date of Delivery’;
- ‘Whether live or still-birth’;
- ‘Name of Institution or Address where Delivered’;
- ‘Name of Health Visitor undertaking the Interview’;
- ‘If interview not obtained given reasons here’;
- ‘Tick here when completed Questionnaire returned by Health Visitor’.
