Description	This file contains the following kinds of circular letters and blank forms, used in the MRC National Survey of Health and Development (NSHD) between 1962 and 1992: - address update forms; - consent forms for hospitals to share medical records of NSHD study members; - questionnaires relating to survey members admitted to hospital; - letters to hospitals from the NSHD, requesting data of their study members; - copies of letters sent from the Chief Medical Officer to regional Medical Officers, in support of the NSHD; - thankyou letters to medical and educational officers; - confidentiality and consent forms for NSHD study members; - letters inviting study members to participate in the latest round of data collection.
