File contains: - a ‘Pilot Postal Questionnaire’ booklet; - correspondence with a doctor, to test the questionnaire on about 50 people, aged between 55 and 65, at their practice; - a ‘Pilot Pre Assessment Booklet 2006’ booklet; - feedback from participants (anonymised).
