Title	2015 data collection: Pack for NSHD study members

Description	The unused contents of a pack sent to study members of the MRC National Survey of Health and Development (NSHD), as part of the 2015 NSHD data collection. The pack contains: - a consent booklet; - an ‘activity monitor time sheet’ booklet, with separate instruction sheet for the activity monitor; - a measurement card for different health measurements; - a checklist; - several pre-paid envelopes. The pack would also have contained: - a ‘self-completion booklet’; - a ‘paper tests’ booklet; - an ‘activity questionnaire’ booklet.
